Adoptive Parents:

If the adoptee is under the age of 18, or over the age of 18 and signs a notarized consent form, Adoptive Parents can:

Update the adoptee’s files with personal information and their consent for contact with the adoptee’s birth parents should they contact Catholic Charities. There is no fee for this service.

Request that the adoptee’s birth family records be reviewed for a signed release. There is a $50 fee for this service.

Request non-identifying medical information about the adoptee’s birth family contained in the record. The fee for this service is $150.

Request a non-identifying summary of information from the adoptee’s birth parent’s file. This could include medical information, demographics, education level, physical appearance, etc. The fee for this service is $300.
